The Alpha Psi Omega Constitution was revised at the National Meeting in 2009, the revised version of the Delta Psi Omega Constitution is still in the works so the 1997 edition is provided here.

Provided as the starting point of each Chapters' local Constitution, this is the 1997 revision of our template.

Student Member Scholarship Nomination Form

Alpha Psi Omega awards two one-time scholarships to student members annually, currently in the amount of $750.

It is required that new members be reported to the National Office within 2 weeks of their Induction. We would prefer the form be typed and printed, so we are providing it as a Word document. It is even permissible to email the report to the National Business Manager, however payment of dues must also be sent in. (And we're not quite ready to do that online.)

Induction Card

Formerly known as the 'Bid Card' or 'Pledge Card', this is the standard form for inviting a potential new member to join. Chapters are welcome to use this or create their own, but the information on the card must be provided to the prospective members at this time.

Supplies Order form

The form to order supplies from the National Business Manager. Please print out a copy and mail the filled out form and payment to the address listed on the form.
