You fill out the form available at http://www.alphapsiomega.org/images/Documents/InitiationForm.pdf (or http://www.alphapsiomega.org/images/Documents/InitiationForm.doc if you want to fill it out electronically first), get that signed by your advisor (Dr. Doll, if I am not mistaken?), and the amount and address to send it to is on the form.

Just to put it in this post, it's $30/new initiate, and it gets mailed to Dr. Bret Jones at his office at Wichita State University in Kansas.
